Young Citizens Award Nomination Form

The Young Citizens Awards recognize citizens (20 years or younger) who have been nominated by the community for demonstration of exceptional commitment to the principles of good citizenship and community involvement.  It is important that the reasons for the nomination are clearly stated in the information presented by the nominator. Nominators are asked to limit their nominations to two outstanding candidates.  Nominator should clearly detail the reasons for the nomination, along with the reason the applicant fits into the selected category.  The nomination forms will be reviewed by a jury of select members of the Chamber’s Business & Youth Committee.

Please attach typed or PRINTED information detailing all of the reasons the candidate should be honored as an outstanding Young Citizen.  Be specific and list ALL details of exceptional service and activities that support the nomination.  Please include any substantiating information, such as letters and press clippings.
